Community Care Membership is a ONE YEAR Subscription

The patient's Community Care enrollment fee is due the day the agreement is signed and covers a period of one year from the day the agreement is signed. This annual enrollment fee may be paid annually or in 12 monthly payments. If the member choose the monthly payment schedule, then the member will have the monthly fee withdrawn from the a bank account on the first or the fifteenth of the month, whichever is closer to the date of the signed agreement.  

The Cost to You

Adult Individual --                     Annual Fee: $600  OR  Monthly Fee: $50
Child 17 and under --               Annual Fee: $480  OR   Monthly Fee: $40
Multiple children per child --  Annual Fee: $420  OR  Monthly Fee: $35

There is also a one-time set-up fee of $25 per adult when enrolling in the DeBruin Medical Community Care discounted concierge medical program.  

Community Care Membership is NOT Insurance

The Community Care Discounted Concierge Enrollment Fee is not a cover benefit under any insurance company, including Medicare. Insurance companies will not pay the fee. It is the sole responsibility of the patient.

Medical services included in the program are clearly laid out (see Description and Services) for the annual cost stated above. There will never be any additional cost for the stated included services.

This program is not in lieu of insurance. Ideally, you will still need health insurance to cover medical costs not included in the annual fee. Health insurance will be used for care you need outside of the office for such services as, but not limited to, specialists, hospitals, labs, x-rays, and pharmacies. If you do not have any health insurance, then the member will be held responsible for all incurred charges.
